Dishwasher cutlery baskets are usually a plastic mesh basket which is usually in one piece, with perhaps just the handle as a separate spare part although these are normally supplied with the basket and not separately.
The most common thing that causes a cutlery basket to break is for people to drop knives and other heavy items of cutlery into the basket which, over time, slowly stresses the bottom of the basket.
Eventually the plastic cracks and allows items to fall through. It is important to replace the cutlery basket when this happens as items that drop through like that can stop the lower spray arm from properly turning causing poor wash results and leaks.
